Adults Support Broker ***

We are working in partnership with a Local Authority in the Sutton area who are seeking a Support Broker to work within the Adult Social Care Service.

Contract - 12 months with a view to extend to an ongoing contract.

Full Time Hours - 36 Hours per week.

Location - SM1. (Hybrid working may be available)

Rate of Pay - £17 - £20 per hour - Negotiable depending on experience.

Main Duties:

  • Support Brokers arrange a range of care services for customers in their own homes or in bed based residential care at the request of Social Workers.
  • The role entails collaborative working with Sutton's Social Locality Work Teams, Commissioning Team, START Team, Hospital Discharge Team, Safeguarding and other Council's internal teams in ensuring a smooth transition for customers in need of support or care.
  • External relationships are with the voluntary sector, care providers, service users and their families.
  • Using Social workers support plans Brokers arrange packages in negotiation with service users or their representatives in ensuring their wishes and needs will be met.

Specific Accountabilities:

  • Manage the throughput of work via a duty system that includes telephone enquiries, emails and direct referrals via the Social services client record system Mosaic.
  • To be responsible for the assessing of customer's circumstances and identify any potential barriers in accessing appropriate services.
  • Find and facilitate access to accommodation and support as appropriate to assessed needs in support plans.
  • Help build a thorough knowledge of all services available in order to offer the best advice to customers to meet their needs.
  • Refer quality or safeguarding issues to the manager or appropriate team or agency.
  • Ensure accurate records are kept in helping meet the Performance Indicators set
  • Carry out duties and ensure practice meets Sutton Council's Equality and Diversity Policy.

Experience/Knowledge:

  • Knowledge of the 2014 Care Act.
  • Knowledge of the 2005 Mental Capacity Act and the added amendment the Deprivation of Liberty Safeguards (DoLS).
  • Significant experience of the Care Sector.
  • Knowledge of the Safeguarding legislation and associated policies.
