CHC Nurse Assessor (Children and Young People)
Temporary contract for 6 weeks initially
Full time - 36 hours, Monday-Friday
£550/day (Inside IR35)
Located in Croydon
We are recruiting a CHC Nurse Assessor for our Local Authority client in Croydon. You'll be responsible for completing the CHC assessments for children and young people.
Key responsibilities:
- Conduct comprehensive Children and Young People's Continuing Care assessments in line with the latest Department of Health guidelines (Feb 2016).
- Evaluate the healthcare and social care needs of children and young people, ensuring that the assessments are thorough, accurate, and meet the necessary criteria for continuing care.
- Work in collaboration with social care teams, healthcare providers, families, and other relevant stakeholders.
- Maintain up-to-date and accurate records of assessments and decisions, ensuring compliance with regulatory standards.
- Provide expert advice and support in relation to Continuing Care for Children and Young People.
Requirements:
- Registered with NMC (Nursing and Midwifery Council).
- Extensive experience in paediatrics or learning disability care, with a focus on working with children and young people.
- In-depth knowledge and understanding of the Children and Young People's Continuing Care Guidelines (Dept of Health, Feb 2016).
- Proven experience of completing Children and Young People's Continuing Care assessments in line with the guidelines.
