Band 6 Registered Dementia Practitioner

Spencer Clarke Group are looking for a Registered Dementia Practitioner to work as part of a dementia assessment team within the community, managing a caseload autonomously with support and supervision. You will be assessing new referrals, liaising with clients, carers, and families, and actively contributing to multidisciplinary team meetings

Role Responsibilities:

• Conduct assessments of new referrals to the service, gathering relevant information from clients, carers, and families to inform care plans and interventions
• Deliver high-quality mental health assessments, with a particular focus on dementia-related conditions
• Participate in regular MDT meetings, contributing to collaborative discussions around client care and ensuring holistic approaches to care planning.
• Assess and diagnose dementia-related conditions and other mental health concerns in older adults.

Essential Requirements:

• Recent experience (within the past 2 years) in a dementia assessment team or memory clinic setting.
• A qualified mental health practitioner (e.g., RMN, Social Worker, OT) with a professional qualification.
