Job Title: Occupational Therapist (Adult Social Care)

Care home Therapy Service

Full time locum contract

£31.01p/h

Located in Southwark

Our Local Authority client are recruiting a locum Occupational Therapist to join their service in Southwark that aims to transform the care home pathway and improve outcomes for residents.

As part of the care home therapy service, you'll support discharges from hospital into local care homes, and promote the ongoing well-being and independence of residents. The service will cover seven larger care homes and several smaller homes within the borough.

Key Responsibilities:

• Deliver high-quality case management and therapy to older adults, particularly those living with dementia.
• Support safe and appropriate discharges from hospital into care homes.
• Assess and address the occupational, postural, and seating needs of residents.
• Work collaboratively with hospital discharge teams, care home staff, GPs, and other health and social care professionals.
• Provide coaching, guidance, and support to care home staff to enhance day-to-day practices and outcomes.

Requirements:

• HCPC Registration and relevant qualifications in Occupational Therapy.
• Proven experience working within a local authority setting, particularly in adult social care.
• Experience of working with people with disabilities, including 1:1 work, the identification of needs and planning and implementing treatment
