Social Care Assistant (Adults) **

We are working in partnership with a Local Authority based in the Bexley area that are seeking a Social Care Assistant to work within the Reablement team to carry out Care Act assessment, equipment provision and rehabilitation.

Main purpose of the job:

  • Assess the needs of adults and older people with physical disabilities, mental health , and/or learning disabilities and their informal carers in accordance with the Care Act and eligibility criteria
  • Work collaboratively with other health and social care colleagues to facilitate an integrated approach and seamless support to service users.

Contract Lengths: 3-6 months with a view to extend to an ongoing contract.

Pay Rates: £21+ per hour (negotiable depending on your experience)

Hours: 35 Hours per week.

Location: DA6.

Major Duties and Responsibilities:

  • To assess the needs of older people, including those with mental health needs , people with physical disabilities, and their carers needs, within the community following their discharge from hospital under the discharge to assess process.
  • As part of a multi-disciplinary team you will ensure client and carers' wishes are central to the decision making and together formulate and implement appropriate care plans which will maximise their independence and help them to remain living in the community for as long as possible, or assist them to move seamlessly into more appropriate accommodation.
  • To work collaboratively with multi- disciplinary teams both in the community and or within the hospital setting in facilitating timely and effective care and support plans to maximise independence at home or to return home with 'own bed is best' ethos including the development and monitoring of reablement programmes.

Experience Needed:

  • Demonstrable relevant paid or voluntary experience of working with older people, adults with disabilities and carers.
  • Experience of dealing with members of the public in context of Health or Social Care.
