Spencer Clarke Group are seeking a Community Care Officer for a local authority in Nottinghamshire.

In this role, you will assess vulnerable adults, develop care plans, manage caseloads and coordinate support services.

Duties:

  • Conduct adult and carer assessments.
  • Develop and review person-centred care plans.
  • Manage caseloads and assess safeguarding risks.
  • Coordinate care services and maintain case records.

Qualifications and Experience:

  • Experience in Adult Social Care.
  • Experience conducting adult and carer assessments.
  • Experience managing caseloads and safeguarding risks.
  • Experience developing and reviewing care plans.

What's on offer:

Salary: £21ph

*may negotiate higher for exceptional candidates, based on experience*

Contract type: 4-6 months minimum

Hours: Monday to Friday, 37 hours per week
