Family Help Lead Practitioner

We are currently working in partnership with a Local Authority in the Ellesmere Port area to recruit an experienced Family Help Lead Practitioner to join their Children and Families service.
This is an excellent opportunity for a skilled practitioner who is passionate about improving outcomes for children, young people and families through early intervention, whole-family working and multi-agency collaboration.

As a Family Help Lead Practitioner, you will play a key role in delivering Family Help services that safeguard and support children and young people.

You will undertake assessments, coordinate support plans, deliver evidence-based interventions and work closely with families and partner agencies to ensure children can thrive in safe and supportive environments.

Working within a strengths-based, trauma-informed framework, you will champion the voice of the child and help families achieve sustainable positive outcomes.

Key Responsibilities

• Undertake holistic assessments of children, young people and families to identify needs and risks.
• Develop, coordinate and review effective support plans that achieve positive outcomes.
• Work in partnership with schools, health services and other agencies to deliver multi-agency interventions.
• Chair professional meetings and ensure clear actions, decisions and records are maintained.
• Deliver evidence-based interventions and promote whole-family working approaches.
• Advocate for children and families, helping to reduce barriers to support services.
• Maintain accurate case records and contribute to safeguarding and child protection processes where required.

Key Requirements

• Qualified Social Worker (Social Work England registered) or relevant Level 3 Childcare qualification with significant experience working with children and families.
• Experience undertaking assessments and managing family support cases within children's services.
• Strong knowledge of safeguarding, child protection procedures and relevant legislation.
• Understanding of the Families First Partnership framework and early intervention principles.
• Excellent communication, assessment and report-writing skills.
• Ability to build effective relationships with families, professionals and partner agencies.
• Enhanced DBS clearance and the ability to manage a varied workload in a fast-paced environment.

Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £17.85 PAYE
Working pattern - 37 hours per week, onsite.
