We are currently recruiting for a Leaving Care Personal Advisor to support young people aged 18-25 who are transitioning from care to independent adulthood. Working as part of a dedicated team in the Darlington area, you will manage a caseload of care leavers, providing practical, emotional and outcome-focused support to help them achieve positive futures.


The successful candidate will work in line with relevant leaving care legislation and local support frameworks, regularly visiting young people, reviewing Pathway Plans, maintaining accurate records, and helping young people develop the skills and confidence needed for independent living. You will also work jointly with Social Workers to support young people aged 16-17 in preparation for leaving care.


Key Responsibilities

  • Manage a caseload of care leavers aged 18-25.
  • Develop, review and update Pathway Plans to meet individual needs and aspirations.
  • Conduct regular visits and maintain positive professional relationships with young people.
  • Provide advice and support around accommodation, education, employment, training, health and wellbeing.
  • Maintain accurate and up-to-date case records using Liquid Logic.
  • Work collaboratively with Social Workers and partner agencies to achieve positive outcomes.
  • Co-work with 16-17-year-olds and their Social Workers to prepare them for leaving care and greater independence.
  • Participate in team meetings, duty cover, supervision sessions and travel as required as part of the role


Key Requirements

  • Experience working with care leavers, vulnerable young people, or within children's services.
  • Sound understanding of leaving care legislation and statutory responsibilities.
  • Ability to build effective relationships and engage positively with young people.
  • Experience of completing assessments, support plans or Pathway Plans.
  • Strong written and verbal communication skills.
  • Ability to maintain accurate case recordings and use electronic case management systems.
  • Excellent organisational skills with the ability to manage a varied caseload.
  • Full UK driving licence and access to a vehicle, with willingness to travel as required.


Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £16.62 PAYE
Working pattern - 37 hours per week.
