Spencer Clarke Group are seeking a Social Care Facilitator for a local authority in Walsall.
In this role, you will undertake Care Act assessments, manage a caseload, develop person-centred support plans, and work with adults, carers and partner agencies to achieve positive outcomes.
Duties:
- Complete Care Act needs and carers' assessments.
- Develop and review person-centred support plans.
- Manage a caseload and maintain accurate records.
- Coordinate services with adults, carers and partner agencies.
Qualifications and Experience:
- Strong experience carrying out Care Act assessments
- Proven experience managing adult social care caseloads
- Experience developing person-centred care and support plans
- Experience working with adults, carers and multi-agency teams
What's on offer:
Salary: £18ph
*may negotiate higher for exceptional candidates, based on experience*
Contract type: 6 months minimum
Hours: Monday to Friday, 37 hours per week
