Our Local Authority client in Yorkshire is looking for a Personal Advisor to join our Leaving Care Service and support young people as they make the transition from care into adulthood. This is a rewarding role where you will build trusted relationships with young people and work alongside them to develop and review Pathway Plans that support their journey towards independent living

Essential Requirements:

• At least 2 years' experience working with adolescents/young people
• Ability to undertake objective assessments and develop effective support plans
• Enhanced DBS check - Children

Duties include:

• Develop, coordinate and review Pathway Plans with young people
• Help young people access housing, education, training, employment, health and financial support
• Develop positive working relationships with partner agencies and contribute to improving services for care leavers
