Leaving Care Worker - East of England LA
£27.33 per hour PAYE
3-6 months with the view of extension
Hybrid - 2 days onsite - 3 days remote
We are looking for a dedicated and motivated Leaving Care Worker to support young people aged 16-25 who are transitioning from care into independent adult life. You will support young people to achieve positive outcomes in education, training and employment, secure and maintain suitable accommodation, improve their health and wellbeing, and develop the confidence and practical skills needed for independent living.
Essential Requirements:
- Experience of working with young people, particularly those with complex needs or who are care-experienced.
- Understanding of the challenges faced by care leavers transitioning into adulthood.
- Ability to build positive relationships and engage young people who may be reluctant to access support.
Duties include:
- Build and maintain trusting, supportive relationships with young people aged 16-25, offering consistent and reliable guidance throughout their transition to adulthood
- Coordinate, implement and review individual pathway plans and other relevant support plans, ensuring young people's needs, wishes and aspirations remain at the heart of their support
- Work in partnership with children's services, adult services, housing, education, employment, health services and other agencies to coordinate effective, joined-up support
