Care Assessor

We are currently working with a Local Authority client in the Northwest London area who is looking to recruit an experienced Care Assessor to join their Learning Disabilities Support Planning Team.

In this role you'll:

• Assess adults with learning disabilities and complete person-centred support plans.
• Manage, review, and monitor care packages, ensuring services meet individual needs.
• Work closely with families, carers, health professionals, and partner agencies to achieve positive outcomes.
• Support safeguarding processes and contribute to multi-disciplinary assessments and reviews.

Essential experience and skills:

• Experience completing social care assessments and support plans.
• Knowledge of learning disabilities, safeguarding, and adult social care legislation.
• Experience working with vulnerable adults within a local authority, NHS, or similar setting.
• Strong communication skills and the ability to work effectively with partner agencies and families.

Contract Length: 3-6 months with the view to extend
Pay Rate: £22.41 PAYE
Working pattern - 37 hours per week, Hybrid working arrangements.
