Spencer Clarke Group are seeking a Social Work Officer for a local authority in North London.
In this role, you will support the safe and timely discharge of adults from hospital by coordinating care, completing assessments and working closely with health and social care partners.
Duties:
- Support safe and timely hospital discharges.
- Complete assessments, reviews and support planning.
- Coordinate care packages, placements and community services.
- Liaise with patients, families, carers and multidisciplinary teams.
Qualifications and Experience:
- Experience working in a fast-paced Hospital Discharge Team.
- Experience supporting Care Act assessments and discharge planning.
- Experience working with Mental Capacity Act and safeguarding processes.
- Experience coordinating with health, social care and community providers.
What's on offer:
Salary: £21ph
*may negotiate higher for exceptional candidates, based on experience*
Contract type: 3-6 months minimum
Hours: Monday to Friday, 36 hours per week
